What Causes Appendicitis?

Appendicitis typically occurs when the opening of the appendix (the lumen) becomes blocked. This blockage prevents the appendix from draining properly, leading to swelling, infection, and pus buildup. Various factors can cause this blockage, including:

• Fecaliths (hardened stool or fecal matter)
• Lymphoid hyperplasia (enlargement of lymphatic tissue within the appendix)
• Digestive tract infections (e.g., viral, bacterial, fungal, or parasitic)
• Tumors (cancerous or noncancerous growths) in the appendix or large intestine
• Appendicoliths (firm, packed stool and mineral deposits in the appendix)
• Abdominal trauma
• Swallowed foreign objects (rare), such as fruit seeds (e.g., from oranges, melons, or grapes)

Once the appendix is blocked, it becomes swollen, painful, and filled with pus.

What is Pus?

Pus is a thick, yellowish-white fluid that consists of bacteria, dead white blood cells (the body’s infection fighters), and tissue debris.

The accumulation of pus in the inflamed appendix increases pressure within the organ, which can disrupt its blood supply.

This may lead to necrosis (tissue death) and, if left untreated, can cause the appendix to rupture or burst open.

If the appendix ruptures, the pus may either form an abscess (a pocket of pus and bacteria) around the appendix or leak into the abdominal cavity, leading to a serious, potentially life-threatening infection called peritonitis.

Can an Appendix Rupture?

It’s not always possible to predict whether an inflamed appendix will rupture, but certain factors can increase the likelihood. Research suggests that appendicoliths (hard stool and mineral deposits) are more commonly associated with rupture than other types of blockages, such as fecaliths.

People at higher risk for appendix rupture include those who:

• Are over 60 years old
• Have a fever above 99.1°F
• Show signs of guarding (tensing of the abdominal muscles when pressure is applied to the painful area)
• Have a high white blood cell count
• Experience pain lasting more than 24 hours

Prompt diagnosis and treatment are crucial to prevent complications like rupture and infection.
